Carers Afternoon – Wednesday 29 August 2018

The Wembley Practice will be hosting a Carers Health & Wellness Afternoon for all members of the public on Wednesday 29 August 2018  between 1.00pm and 4.00pm – all are welcome to attend. See poster below.

The event aims to allow our patients who identify as carers to experience a number of local health and social care service providers accessible through the surgery for our patients as well as other residents of Brent.

Although this event is aimed at carers, we encourage all of our patients to meet the local healthcare services.

Visitors will also have the opportunity to meet PPG representatives and members of practice staff, particularly newer team members, and members of Brent CCG.

We are also pleased to announce that we will be joined by the mayor of Brent, as well as a member of the Adult Social Care Team for Brent Council.

The services due to attend are:

  • Brent Carers, who have been supporting unpaid adult and young carers since 1994. Their vision is to ensure that unpaid carers are recognised, valued and supported to live.
  • Healthwatch Brent, who help local people get the best out of their health and social care services by running various events, projects and activities in and around Brent.
  • Brent Integrated Diabetes Service, who strive to deliver high quality specialist diabetes care in the community and closer to the homes of patients.

A number of other services will also be present for the whole family to benefit from.

AT Medics practices schedule regular PPG meetings with SMS reminders, dedicated PPG notice boards and a translator if necessary, and are delighted to be hosting this event at The Wembley Practice.

The Wembley Practice is committed to delivering high quality care focused on patient needs and safety, and is working to achieve clinical improvement through the implementation and advancement of quality systems, as well as continually reviewing and investing in training and education of staff.
